Job Description
We are seeking a dedicated and compassionate Rehabilitation Nurse to join our esteemed team in Rockford, IL. This full-time position offers a unique opportunity to make a profound difference in the lives of patients recovering from various injuries, illnesses, or surgeries. As a Rehabilitation Nurse, you will play a crucial role in developing and implementing individualized care plans, focusing on restoring function and promoting independence. Key Responsibilities
- Assess, plan, implement, and evaluate patient care plans in a rehabilitation setting.
- Collaborate with an interdisciplinary team (physicians, therapists, social workers) to optimize patient outcomes.
- Educate patients and their families on rehabilitation techniques, medication management, and self-care strategies.
- Administer medications and treatments as prescribed, monitoring for effectiveness and adverse reactions.
- Document patient progress, observations, and interventions accurately and thoroughly.
- Advocate for patients' rights and ensure a safe, therapeutic environment.
- Participate in discharge planning and coordinate post-discharge care.
- Provide emotional support and encouragement to patients throughout their recovery journey.
Required Skills
- Current Registered Nurse (RN) license in the State of Illinois (or eligible for licensure).
- At least 1 year of clinical nursing experience, preferably in a hospital or rehabilitation setting.
- Strong knowledge of rehabilitation principles and patient care techniques.
- Excellent communication, interpersonal, and critical thinking skills.
- Ability to work effectively in a fast-paced, team-oriented environment.
- BLS certification required; ACLS preferred.
- Demonstrated commitment to patient-centered care.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Certified Rehabilitation Registered Nurse (CRRN) certification.
- Experience with electronic health record (EHR) systems, particularly Epic.
- Previous experience working with diverse patient populations and cultural backgrounds.
