Job Description
We are seeking a dedicated and compassionate Hospice Intake Coordinator to join our team in Miami. This crucial administrative role serves as the initial point of contact for patients, families, and healthcare providers seeking hospice services. You will be responsible for managing the intake process from referral to admission, ensuring all necessary documentation is collected, verified, and processed accurately and efficiently. The ideal candidate will possess exceptional organizational skills, a strong ability to communicate with empathy and clarity, and a deep commitment to supporting individuals and families during a sensitive time.
Key Responsibilities
- Serve as the primary point of contact for all hospice referrals, inquiries, and admissions.
- Conduct initial screenings to determine patient eligibility for hospice services based on established criteria.
- Collect and verify comprehensive patient demographic, medical, and insurance information.
- Liaise effectively with referring physicians, hospitals, and other healthcare facilities to obtain necessary medical records and orders.
- Educate patients and families about hospice services, benefits, and the admission process with sensitivity and clarity.
- Coordinate initial assessments and schedule visits for clinical staff.
- Maintain accurate and confidential patient records in accordance with HIPAA regulations and company policies.
- Process all intake paperwork, including consent forms, insurance authorizations, and financial agreements.
- Collaborate closely with the interdisciplinary hospice team to ensure a seamless transition for new patients.
- Provide administrative support to the hospice department as needed.
Required Skills
- Minimum of 1 year of administrative or patient intake experience, preferably in a healthcare setting.
- Exceptional communication and interpersonal skills, with a strong emphasis on empathy and active listening.
- Proficiency in medical terminology and understanding of healthcare processes.
- Excellent organizational skills and attention to detail.
- Ability to multitask and manage time effectively in a fast-paced environment.
- Strong computer skills, including experience with electronic medical records (EMR) systems and Microsoft Office Suite.
- Demonstrated ability to maintain confidentiality and adhere to ethical standards.
- A compassionate and patient-centered approach to care.
Preferred Qualifications
- Associate's degree or higher in healthcare administration, business, or a related field.
- Previous experience specifically in hospice or home health intake.
- Knowledge of insurance verification and authorization procedures relevant to hospice care.
- Bilingual in English and Spanish.
