Job Description

We are seeking compassionate and dedicated individuals to join our Palliative Care Support team in Ballarat. This rewarding full-time role involves providing essential physical, emotional, and social support to clients and their families, enabling them to live with comfort and dignity during a challenging time. You will work within a supportive multidisciplinary team, delivering person-centred care and contributing to a positive and peaceful environment. Key Responsibilities

  • Provide respectful and person-centred care, assisting clients with daily living activities.
  • Offer emotional support and companionship to clients and their families.
  • Monitor and report changes in clients' condition to nursing staff or healthcare professionals.
  • Maintain a clean, safe, and comfortable environment for clients.
  • Assist with mobility, transfers, and light domestic duties as required.
  • Document care provided accurately and in a timely manner.
  • Adhere to all policies, procedures, and safety guidelines.
  • Collaborate effectively with the multidisciplinary team to ensure holistic care.

Required Skills

  • Excellent communication and interpersonal skills.
  • Demonstrated empathy, compassion, and patience.
  • Ability to work independently and as part of a team.
  • Strong observational and reporting skills.
  • Current First Aid and CPR certification.
  • Valid Australian Driver's License and access to reliable transport.
  • Clear National Police Check and Working with Children Check (or willingness to obtain).

Preferred Qualifications

  • Certificate III or IV in Individual Support (Aged Care, Disability, Home and Community Care).
  • Previous experience in palliative care, aged care, or a similar support role.
  • Knowledge of palliative care principles and end-of-life care.
