Job Description
We are seeking a highly experienced and dedicated NDIS Support Coordinator – Specialist to join our dynamic team in Osborne Park. In this pivotal role, you will provide advanced, specialised support coordination services to NDIS participants with complex needs, ensuring they can effectively implement their NDIS plans. This involves navigating the NDIS framework, connecting participants with appropriate services, building capacity, and advocating on their behalf to achieve their goals and enhance their independence. Your expertise will be crucial in developing innovative solutions and fostering strong relationships with participants, families, and service providers.
Key Responsibilities
- Provide high-level, specialist NDIS Support Coordination to a caseload of participants with complex support needs.
- Assist participants in understanding and implementing their NDIS plans, ensuring optimal utilisation of funded supports.
- Identify, link, and coordinate a range of formal and informal supports that align with participants' goals and cultural backgrounds.
- Develop and maintain strong working relationships with participants, their families, carers, and other stakeholders.
- Monitor the implementation of support plans, review progress, and facilitate plan reviews as required.
- Advocate on behalf of participants to resolve service delivery issues, navigate system complexities, and ensure rights are upheld.
- Maintain accurate and timely case notes, records, and reports in accordance with organisational policies and NDIS guidelines.
- Contribute to the continuous improvement of support coordination services through feedback and active participation in team meetings and professional development.
Required Skills
- Minimum of 2 years of experience in NDIS Support Coordination, with a focus on complex cases.
- Demonstrated expert knowledge of the NDIS Act, Price Guide, Quality and Safeguards Framework, and operational guidelines.
- Proven ability to manage a diverse caseload, prioritise tasks, and meet deadlines.
- Exceptional communication, negotiation, and interpersonal skills, with the ability to build rapport quickly.
- Strong problem-solving and critical thinking abilities.
- Proficiency in using client management systems and Microsoft Office Suite.
- Valid Australian Driver's License and access to a reliable vehicle.
Preferred Qualifications
- Tertiary qualification in Social Work, Disability Studies, Allied Health, Community Services, or a related field.
- Experience working with a diverse range of disability types and cultural backgrounds.
- Knowledge of local service providers and community resources in the Perth metropolitan area.
