Job Description
We are actively seeking compassionate and dedicated Rehabilitation Nurses to join our dynamic team. This critical role focuses on helping patients regain independence and improve their quality of life after illness, injury, or surgery. As a Rehabilitation Nurse at LHSC, you will work within a multidisciplinary team to develop and implement individualized care plans, provide direct nursing care, and educate patients and their families. Key Responsibilities
- Assess patient needs and develop individualized rehabilitation nursing care plans.
- Provide direct patient care, including medication administration, wound care, and managing complex medical conditions.
- Monitor and evaluate patient progress, adapting care plans as necessary.
- Collaborate with physicians, physical therapists, occupational therapists, speech-language pathologists, and social workers.
- Educate patients and their families on self-care techniques, disease management, and rehabilitation goals.
- Facilitate patient mobility, functional independence, and activities of daily living.
- Document all aspects of patient care accurately and comprehensively.
- Ensure a safe and therapeutic environment for patients.
- Participate in team meetings and contribute to care planning discussions.
Required Skills
- Current registration as a Registered Nurse (RN) with the College of Nurses of Ontario (CNO) or eligibility to register.
- Demonstrated experience in acute care or a rehabilitation setting.
- Strong assessment, critical thinking, and problem-solving skills.
- Excellent communication and interpersonal abilities.
- Ability to work effectively as part of a multidisciplinary team.
- Commitment to patient-centered care and continuous quality improvement.
Preferred Qualifications
- Bachelor of Science in Nursing (BScN).
- Certification in Rehabilitation Nursing (CRN(C)) or willingness to obtain.
- Experience with electronic health record (EHR) systems.
- Proficiency in a second language relevant to the diverse patient population.
