Job Description

Key Responsibilities
- Provide person-centred care and support to individuals with learning disabilities, autism, mental health conditions, and complex needs.
- Assist service users with daily living activities, including personal care, meal preparation, medication administration, and household tasks.
- Support individuals to access community activities, appointments, and social events, promoting independence and inclusion.
- Develop and maintain positive relationships with service users, their families, and other professionals.
- Adhere to individual care plans, policies, and procedures, ensuring the highest standards of care.
- Maintain accurate records and report any concerns or changes in a timely manner.
- Participate in training and development opportunities to enhance skills and knowledge.
