Job Description
We are seeking a dedicated and compassionate Triage / Telehealth Nurse to join our growing team in Chula Vista, CA. This crucial role involves assessing patient symptoms, providing medical advice, and coordinating care primarily through virtual platforms, ensuring timely and appropriate interventions. You will be a vital link between patients and our medical staff, offering guidance and support. Key Responsibilities
- Conduct comprehensive patient symptom assessments via phone or video conferencing.
- Provide evidence-based medical advice and health education to patients.
- Triage patient calls/messages to determine the urgency of medical needs and direct them to appropriate levels of care (e.g., emergency department, urgent care, primary care, specialist).
- Document all patient interactions, assessments, and recommendations accurately and promptly in the electronic health record (EHR).
- Collaborate with physicians, advanced practice providers, and other healthcare team members to ensure seamless patient care.
- Follow established clinical protocols and guidelines to ensure consistent, high-quality care.
- Educate patients on medication management, chronic disease management, and preventive care.
- Participate in continuous quality improvement initiatives related to telehealth services.
- Maintain strict confidentiality and adhere to all HIPAA regulations.
Required Skills
- Current Registered Nurse (RN) license in California or ability to obtain prior to start date.
- Minimum of 2 years of acute care or clinical experience.
- Strong clinical assessment and critical thinking skills.
- Excellent verbal and written communication skills.
- Proficiency with electronic health records (EHR) and telehealth technology.
- Ability to work independently and manage multiple tasks in a fast-paced environment.
- Compassionate and patient-centered approach to care.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Previous experience in a triage, emergency room, urgent care, or telehealth setting.
- Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).
- Bilingual proficiency (English/Spanish) highly desirable.
- Familiarity with Sharp HealthCare systems and protocols.
