Job Description

We are seeking a dedicated and compassionate Clinical Social Worker to join our integrated healthcare team. This role involves providing comprehensive social work services to patients and their families within a hospital setting, addressing complex psychosocial needs, facilitating discharge planning, and ensuring access to appropriate community resources. The successful candidate will work collaboratively with medical staff, nurses, and other allied health professionals to deliver holistic, person-centred care. You will be instrumental in advocating for patients, assessing their mental health, safeguarding concerns, and supporting their journey towards recovery and independence.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments of patients and families.
  • Develop and implement individualized care plans, focusing on discharge planning and community reintegration.
  • Provide crisis intervention, counseling, and emotional support to patients and families coping with illness, trauma, or loss.
  • Advocate for patients' rights and ensure their access to necessary resources and services.
  • Collaborate effectively with interdisciplinary teams to ensure coordinated and comprehensive patient care.
  • Identify and address safeguarding concerns, escalating as appropriate.
  • Maintain accurate and timely patient records in accordance with professional standards and organizational policies.
  • Participate in team meetings, case conferences, and professional development activities.

Required Skills

  • Degree in Social Work (BA, BSc, or MA)
  • Registered with Social Work England
  • Minimum of 3 years post-qualification experience in a clinical or healthcare setting
  • Strong assessment, planning, and intervention skills
  • Excellent communication and interpersonal abilities
  • Ability to work effectively within an interdisciplinary team
  • Knowledge of relevant legislation and policies (e.g., Mental Capacity Act, Care Act)
  • Proficiency in safeguarding practices for vulnerable adults and children

Preferred Qualifications

  • Experience in an acute hospital setting
  • Specialist training in mental health, palliative care, or safeguarding
  • Familiarity with NHS systems and processes
  • Experience supervising junior staff or students
  • Evidence of continuous professional development
