Job Description
We are seeking a dedicated and compassionate Triage / Telehealth Nurse to join our remote team. This is a unique opportunity to utilize your clinical expertise in a dynamic virtual environment, providing critical patient assessment, advice, and coordination of care. As a Telehealth Nurse, you will be the first point of contact for members, guiding them through their healthcare journey with professionalism and empathy. Key Responsibilities
- Conduct comprehensive telephonic and virtual health assessments for patients experiencing a wide range of symptoms and health concerns.
- Provide skilled nursing advice, health education, and symptom management guidance based on established protocols and clinical judgment.
- Triage patient calls and virtual consultations to determine the appropriate level of care, including urgent care, emergency services, or scheduled appointments.
- Document all patient interactions accurately and thoroughly in the electronic health record system.
- Collaborate with physicians, specialists, and other healthcare providers to coordinate patient care plans and follow-up.
- Educate patients on medication management, preventive care, and chronic disease management.
- Maintain strict patient confidentiality and adhere to all ethical and professional nursing standards.
- Participate in ongoing training and professional development to stay current with best practices in telehealth and nursing.
Required Skills
- Active Registered Nurse (RN) license in good standing (or eligibility for licensure in Pennsylvania).
- Minimum of 2 years of clinical nursing experience in acute care, emergency, or primary care settings.
- Excellent communication and interpersonal skills, with the ability to convey complex medical information clearly and empathetically over the phone/video.
- Strong critical thinking and problem-solving abilities to assess and triage diverse patient needs.
- Proficiency in using electronic health record (EHR) systems and other telehealth technologies.
- Ability to work independently and manage a caseload effectively in a fast-paced, remote environment.
- Demonstrated commitment to patient-centered care and quality outcomes.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN) degree.
- Previous experience in a telehealth or call center environment.
- Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).
- Fluency in additional languages.
