Job Description
We are seeking a dedicated and compassionate Private Duty Nurse to provide exceptional one-on-one care to clients in their homes throughout Swansea. As a Private Duty Nurse, you will play a crucial role in monitoring patient health, administering medications, performing skilled nursing tasks, and collaborating with families and healthcare teams to ensure the best possible outcomes. This role requires a nurse who is not only clinically proficient but also possesses a strong sense of empathy, excellent communication skills, and a commitment to patient advocacy.
Key Responsibilities
- Provide comprehensive skilled nursing care to clients in a home setting, adhering to individualized care plans.
- Administer medications accurately and timely, including injections, oral medications, and topical treatments.
- Monitor vital signs, observe patient condition, and promptly report any changes to the healthcare team and family.
- Perform wound care, ostomy care, catheter care, and other skilled procedures as prescribed.
- Educate clients and their families on health management, medication protocols, and disease prevention.
- Maintain accurate and detailed patient records, including observations, treatments, and care provided.
- Collaborate effectively with physicians, therapists, caregivers, and family members to ensure holistic patient care.
- Ensure client safety and comfort at all times, including fall prevention and maintaining a clean care environment.
- Assist with personal care, mobility, and activities of daily living as needed, while promoting client independence.
Required Skills
- Valid Registered Nurse (RN) license in the UK (NMC registration).
- Minimum of 2 years of nursing experience, preferably in a home health, acute care, or private duty setting.
- Proficiency in administering medications, performing clinical assessments, and managing chronic conditions.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and manage time effectively.
- Compassionate, patient-centered approach to care.
- Basic Life Support (BLS) certification.
Preferred Qualifications
- Experience with electronic health records (EHR) systems.
- Advanced Cardiac Life Support (ACLS) certification.
- Experience working with clients with complex medical needs or chronic illnesses.
- Additional certifications in specialized areas (e.g., wound care, gerontology).
