Job Description

We are seeking a dedicated and experienced Clinical Pharmacist to join our dynamic pharmacy team at Lancashire Teaching Hospitals NHS Foundation Trust. This crucial role involves providing expert pharmaceutical care, optimizing medication use, and contributing to patient safety and positive health outcomes across various clinical specialties within our hospital. You will work collaboratively with multidisciplinary teams, participate in ward rounds, and offer specialist pharmaceutical advice to healthcare professionals and patients. Key Responsibilities

  • Provide specialist clinical pharmacy services to designated wards and clinical areas.
  • Undertake medication reviews, ensuring safe, effective, and cost-effective use of medicines.
  • Participate in multidisciplinary ward rounds and provide pharmaceutical input to patient care plans.
  • Offer expert advice on drug selection, dosing, administration, and monitoring to medical and nursing staff.
  • Contribute to the development and implementation of medicines management policies and guidelines.
  • Educate patients and caregivers on medication use and potential side effects.
  • Monitor and report adverse drug reactions and medication errors.
  • Supervise and mentor junior pharmacy staff, technicians, and students.
  • Engage in continuous professional development and maintain up-to-date knowledge of pharmaceutical practice.

Required Skills

  • MPharm degree or equivalent.
  • Registered with the General Pharmaceutical Council (GPhC).
  • Minimum of 2 years post-registration experience in a hospital clinical pharmacy setting.
  • Demonstrable knowledge of medicines management principles and clinical pharmacy practice.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
  • Strong problem-solving and decision-making abilities.
  • Ability to manage a varied workload and prioritize tasks effectively.

Preferred Qualifications

  • Postgraduate diploma in clinical pharmacy or equivalent.
  • Independent Prescribing qualification.
  • Experience in a specialized clinical area (e.g., cardiology, oncology, critical care).
  • Experience in teaching or mentoring.
