Job Description
Riverside Medical Center is seeking a dedicated and highly skilled Clinical Pharmacist to join our dynamic pharmacy team. This pivotal role involves providing comprehensive pharmaceutical care to patients, collaborating with healthcare providers, and ensuring optimal medication therapy outcomes. Key Responsibilities
- Provide advanced clinical pharmacy services, including medication therapy management, drug information, and pharmacokinetic dosing.
- Participate in interdisciplinary patient care rounds, offering expert recommendations on pharmacotherapy.
- Monitor patient medication profiles for drug interactions, adverse effects, and therapeutic duplications, intervening as necessary.
- Educate patients and healthcare professionals on medication use, potential side effects, and adherence strategies.
- Assist in the development and implementation of hospital policies and procedures related to medication use and patient safety.
- Conduct medication reconciliation and discharge counseling to ensure continuity of care.
- Supervise pharmacy technicians and contribute to their professional development.
- Engage in quality improvement initiatives and participate in pharmacy research projects as appropriate.
Required Skills
- Doctor of Pharmacy (PharmD) degree or equivalent.
- Current Pharmacist license in any US state or eligibility for licensure in New Jersey.
- Demonstrated knowledge of clinical pharmacokinetics, pharmacotherapeutics, and drug information.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced, multidisciplinary team environment.
- Strong problem-solving and critical thinking abilities.
Preferred Qualifications
- Completion of a PGY1 residency program (ASHP-accredited).
- Board Certification in Pharmacotherapy (BCPS) or other relevant specialty.
- Experience with electronic health record systems (e.g., Epic, Cerner).
- Previous experience in an acute care hospital setting.
