Job Description

VON Canada is seeking dedicated and compassionate Hospice & Palliative Care Nurses to join our team in London, Ontario. This vital role focuses on providing holistic, person-centered care to patients and their families facing life-limiting illnesses. As a Hospice & Palliative Care Nurse, you will be instrumental in managing pain and symptoms, offering emotional and spiritual support, and ensuring comfort and dignity throughout the palliative journey. Key Responsibilities

  • Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
  • Administer medications and treatments as prescribed, focusing on pain and symptom management.
  • Provide emotional, psychological, and spiritual support to patients and their families.
  • Educate patients and caregivers on disease progression, treatment options, and comfort measures.
  • Collaborate effectively with an interdisciplinary team, including physicians, social workers, spiritual care providers, and volunteers.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as necessary.
  • Document patient care accurately and comprehensively in accordance with professional standards and organizational policies.
  • Act as an advocate for patients' rights and wishes, ensuring dignity and respect.
  • Participate in family conferences and provide bereavement support as appropriate.
  • Maintain professional competence through ongoing education and professional development.

Required Skills

  • Current registration as a Registered Nurse (RN) with the College of Nurses of Ontario (CNO) or eligibility to register.
  • Minimum of 2 years of nursing experience, with preference for experience in palliative care, oncology, or chronic disease management.
  • Demonstrated strong clinical assessment and critical thinking skills.
  • Exceptional communication and interpersonal skills, with the ability to build rapport with patients and families.
  • Profound empathy, compassion, and sensitivity to the needs of individuals and families facing end-of-life.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Valid driver's license and access to a reliable vehicle.
  • Eligibility for sponsorship (if applicable) and a commitment to meeting all immigration requirements.

Preferred Qualifications

  • Certification in Hospice Palliative Care (CHPCN(C)) or a commitment to obtaining certification.
  • Experience working in a community-based healthcare setting.
  • Knowledge of grief and bereavement support models.
  • Proficiency in a second language relevant to the London, Ontario community.
