Job Description

We are seeking a highly motivated and compassionate Clinical Social Worker to join our multidisciplinary team within Oxford University Hospitals. This role involves providing essential social work services to patients and their families, addressing complex psycho-social needs, and ensuring patient well-being and discharge planning. The successful candidate will work across various clinical settings, offering expert assessment, intervention, and advocacy to support patients through challenging health circumstances.

Key Responsibilities

  • Conduct comprehensive psycho-social assessments for patients and their families.
  • Provide crisis intervention and supportive counseling to patients dealing with illness, trauma, and end-of-life issues.
  • Develop and implement effective discharge plans, liaising with community services and external agencies.
  • Advocate for patients' rights and access to appropriate resources and services.
  • Participate in multidisciplinary team meetings to contribute to holistic patient care planning.
  • Educate patients and families on available resources and support systems.
  • Maintain accurate and timely patient records in accordance with Trust policies.
  • Identify and respond to safeguarding concerns for vulnerable adults and children.

Required Skills

  • Degree in Social Work or equivalent (e.g., CQSW, DipSW).
  • Current registration with Social Work England.
  • Demonstrable experience in a clinical or healthcare setting.
  • Strong assessment and intervention skills.
  • Excellent communication, interpersonal, and team-working abilities.
  • Ability to work autonomously and manage a complex caseload.
  • Knowledge of relevant legislation, policies, and procedures pertaining to social care and healthcare.
  • Proficiency in maintaining accurate and confidential records.

Preferred Qualifications

  • Master's degree in Social Work or a related field.
  • Experience with specific patient populations (e.g., oncology, palliative care, geriatrics).
  • Familiarity with NHS systems and processes.
