Job Description
We are seeking a dedicated and empathetic Public Service Welfare – Community Care professional to join our team in Rockingham. This vital role involves providing direct support and advocacy to vulnerable individuals and families within the local community. You will work to identify client needs, connect them with appropriate services, and empower them to achieve greater independence and well-being. This position requires a proactive approach, excellent communication skills, and a genuine commitment to making a positive difference in people’s lives.
Key Responsibilities
- Conduct initial assessments of client needs and develop individual support plans.
- Provide information, advice, and advocacy to clients regarding available welfare and community services.
- Facilitate referrals to internal and external support agencies, including housing, health, education, and employment services.
- Offer crisis intervention and de-escalation support when required.
- Maintain accurate and confidential client records in accordance with organisational policies and privacy legislation.
- Collaborate effectively with team members, other service providers, and community stakeholders.
- Participate in community outreach activities and local events to promote services and build relationships.
- Monitor client progress and review support plans regularly to ensure effectiveness.
Required Skills
- Minimum 1 year of experience in a social welfare, community services, or related field.
- Demonstrated understanding of the challenges faced by vulnerable populations.
- Excellent interpersonal and communication skills, both written and verbal.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Strong organisational and time management skills.
- Proficiency in basic computer applications (Microsoft Office Suite).
- Current Western Australian Driver's License.
Preferred Qualifications
- A relevant tertiary qualification (e.g., Certificate IV or Diploma in Community Services, Social Work, or Welfare).
- Experience working with diverse cultural backgrounds and Aboriginal and Torres Strait Islander communities.
- Knowledge of local community resources and welfare systems in the Rockingham area.
- First Aid and CPR Certificate.
