Job Description

Key Responsibilities

  • Provide direct, person-centered support to individuals, assisting them with daily living activities.

  • Assist with personal care needs, medication reminders, and mobility support as required.
  • Engage individuals in community activities and social interaction, promoting inclusion.
  • Maintain clear and accurate documentation of services provided and progress made.
  • Collaborate effectively with team members, supervisors, and families to ensure holistic support.
  • Adhere to all organizational policies, procedures, and regulatory standards.
