Job Description

Key Responsibilities
- Provide direct, person-centered support to individuals, assisting them with daily living activities.
- Support individuals in developing life skills, fostering independence, and achieving personal goals.
- Assist with personal care needs, medication reminders, and mobility support as required.
- Engage individuals in community activities and social interaction, promoting inclusion.
- Maintain clear and accurate documentation of services provided and progress made.
- Collaborate effectively with team members, supervisors, and families to ensure holistic support.
- Adhere to all organizational policies, procedures, and regulatory standards.
