Job Description
We are actively seeking a dedicated and compassionate Geriatric Nurse Practitioner to join our esteemed team in Northampton, Massachusetts. The Geriatric Nurse Practitioner will play a pivotal role in assessing, diagnosing, and managing the health needs of our elderly patient population, ensuring a holistic approach to care. You will collaborate closely with a multidisciplinary team to develop individualized care plans, focusing on preventative care, chronic disease management, and promoting healthy aging. This position requires a strong understanding of age-related physiological and psychological changes, as well as a commitment to advocating for geriatric patients’ best interests.
Key Responsibilities
- Conduct comprehensive geriatric assessments, including physical exams, health histories, and psychosocial evaluations.
- Diagnose and treat acute and chronic health conditions common in older adults.
- Prescribe medications and other treatments as appropriate, monitoring patient responses and adjusting care plans.
- Develop and implement individualized care plans in collaboration with patients, families, and the healthcare team.
- Provide patient and family education on disease management, healthy lifestyle choices, and preventative care.
- Order and interpret diagnostic tests, integrating results into patient management.
- Participate in multidisciplinary team meetings to coordinate patient care and transitions.
- Advocate for geriatric patients' needs and preferences, ensuring ethical and patient-centered care.
- Maintain accurate and timely patient records in compliance with all regulatory standards.
Required Skills
- Current Nurse Practitioner license in Massachusetts (or eligibility to obtain)
- Master's or Doctoral degree from an accredited Nurse Practitioner program with a focus on Gerontology or Family NP with significant geriatric experience
- ANCC or AANP certification as a Geriatric or Adult-Gerontology Primary Care Nurse Practitioner
- Strong clinical assessment and diagnostic skills specific to the geriatric population
- Excellent communication, interpersonal, and collaborative skills
- Ability to work effectively in a fast-paced healthcare environment
- Commitment to continuous learning and professional development
Preferred Qualifications
- 2+ years of experience as a Nurse Practitioner, preferably in geriatrics or adult primary care
- Experience with electronic health record (EHR) systems
- Fluency in additional languages relevant to the local community
