Job Description
We are seeking a dedicated and compassionate Public Health Nurse to join our dynamic community health team in Bolton. This vital role involves promoting public health, preventing disease, and addressing health inequalities within the local population. You will work across various community settings, providing direct patient care, health education, and support to individuals and families. Key Responsibilities
- Conduct health assessments and develop care plans for individuals and families.
- Provide health education and promotion activities on topics such as immunisation, nutrition, sexual health, and disease prevention.
- Manage caseloads, including vulnerable individuals and families, ensuring appropriate referrals and support.
- Collaborate with multi-disciplinary teams, local authorities, and community organisations to deliver integrated health services.
- Participate in public health initiatives, screenings, and outreach programs.
- Maintain accurate and confidential patient records in compliance with professional standards and GDPR regulations.
- Advocate for patients' needs and contribute to policy development to improve public health outcomes.
- Undertake home visits and clinic-based appointments as required.
Required Skills
- Valid UK NMC registration as a Registered Nurse.
- A degree in Nursing or Public Health Nursing.
- Strong communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
- Excellent organisational and time management skills.
- Proficiency in using electronic patient record systems.
- A compassionate and non-judgmental approach to patient care.
Preferred Qualifications
- Previous experience in a community or public health nursing role.
- Experience working with diverse populations.
- Knowledge of local public health priorities and initiatives in Greater Manchester.
- Full UK driving license and access to a car for work purposes.
