Job Description
We are seeking a dedicated and compassionate Geriatric Nurse Practitioner to join our robust healthcare team in New Orleans. This critical role involves providing comprehensive primary and specialized care to our elderly patient population. The successful candidate will be responsible for assessing, diagnosing, and treating acute and chronic conditions, focusing on maintaining and improving the quality of life for older adults. Key Responsibilities
- Conduct comprehensive health assessments, including medical history, physical examinations, and psychosocial evaluations for geriatric patients.
- Diagnose and treat common acute and chronic health conditions prevalent in the elderly, managing complex multi-morbidities.
- Develop and implement individualized care plans, including medication management, lifestyle modifications, and referrals to specialists.
- Order, interpret, and evaluate diagnostic tests to identify and assess patient conditions.
- Provide preventive care, health education, and counseling to patients and their families regarding aging, disease management, and wellness.
- Collaborate effectively with a multidisciplinary team of physicians, nurses, social workers, and other healthcare professionals.
- Maintain accurate and timely electronic health records, documenting all patient encounters and care provided.
- Participate in quality improvement initiatives and continuing education to stay current with best practices in geriatric care.
- Serve as a patient advocate, ensuring access to appropriate resources and services.
Required Skills
- Current active Registered Nurse (RN) license in Louisiana or eligibility for licensure.
- Current active Advanced Practice Registered Nurse (APRN) license in Louisiana or eligibility for licensure.
- Master's or Doctor of Nursing Practice (DNP) degree from an accredited program.
- National board certification as a Geriatric Nurse Practitioner (GNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP).
- Strong clinical assessment and diagnostic skills specific to the geriatric population.
- Excellent communication, interpersonal, and empathy skills.
- Ability to work independently and as part of a collaborative healthcare team.
- Proficiency in electronic medical record (EMR) systems.
- Demonstrated commitment to patient-centered care and ethical practice.
Preferred Qualifications
- Previous experience working in a geriatric-specific setting (e.g., long-term care, home health, geriatric primary care).
- Experience with care coordination for complex geriatric patients.
- Familiarity with local community resources and support services for older adults.
- Bilingual proficiency (e.g., Spanish, Vietnamese, French Creole) is a plus.
